UNC Hospice Presents Advance Care Planning: It’s Time to Talk
Join us as we talk about Advance Care Planning, which is a way to let your loved ones and your health care providers know the scope, type of care, and treatments you do and don’t want when you are unable to express your wishes at the end of life. This process ideally includes designating a trusted person (Health care power of attorney) to serve as your voice to make sure your end of life wishes are followed when you cannot speak for yourself.

We will have an overview of Advance Care Planning, blank documents will be available to complete and a Notary Public will be available to notarize completed documents.
